1. Appointment based on first come first serve. All date and slot appointment are LIMITED.
2. Appointment can be made at least three (3) days and three months before and depend on availability.
3. THE SYSTEM WILL ONLY LIST AVAILABLE DATE ACCORDING TO SELECTED SLOT.
4. Applicant who misused the system will be BLOCKED from make an appointment.
5. Applicant must bring complete document as required (ORIGINAL AND COPIES) along with appointment slip.
6. Check and print appointment slip from SEARCH MY APPOINTMENT menu and bring during appointment.
7. Applicant is responsible for the authenticity of the information provided by the applicant. The Department have rights to check,verify,reject or cancel the appointment for any abused of information.
8. The Department NEVER appoint any THIRD PARTY to book an appointment. The appointment is FREE OF CHARGE.
9. If you receive this notification from the system 'You already have an appointment with us', please follow your appointment date. You can check your appointment details using SEARCH MY APPOINTMENT.
